Radial nerve paralysis associated with humeral shaft fracture is the most common peripheral nerve lesion complicated by fracture and the resulted disability is significant, especially in hand grasping and power gripping.
We experienced a case of high level of radial nerve paralysis associated with humeral shaft fracture and managed it by tendon transferring ; flexor carpi ulnaris transfer to extensor digitorum communis, pronator teres transfer to extensor carpi radialis brevis, and palmaris longus transfer to extensor pollicis longus. The result was satisfactory the pinch and grasping power was a little bit under normal but the range of joint motion was almost normal.
